Frequently Asked Questions

We will need your records from your current provider and once we receive them, we will call you to make an appointment.  Please visit the New Patient page for more information than call the office to see how far out we are currently booking New Patient appointments.

A new patient appointment consists of both the patient and provider getting to know each other.  The Medical Assistant and Provider will review your past medical history including any surgeries, current medications, any family history medical concerns, and any current health concerns or the main reason for your visit.

If you need to cancel your appointment please call the office to reschedule.  Appointments cancelled the day of your appointment will incur a NO SHOW FEE.

Yes there is.  If you miss your appointment or cancel on the same day, a No Show Fee will be applied to your account.  It is impotant to know that patients who miss their appointments are taking away from their own health and also effecting others who may have needed an appointment but we were full.

We do not call patients prior to their appointment to remind them.  If you schedule your next appointment at our office, you will be handed an Appointment Card.  The scheduling software that we use will send out a text the day prior to remind you if you have a cell phone number in our system.  This is not guaranteed so please remember that it is the Patient’s responsibility to remember when their appointments are.

Yes you can!  You will need access to your Patient Portal.  Please click the Patient Portal link or call the office if you have questions on how to do this.

Please stop in the office to fill out a Records Request Form or call with any questions.

There are several ways to do this, either stop in our office to fill out a form and we can Fax/email to your prior Physician’s office or you can call them yourself and ask what their policy is to transfer your records.

No you do not.  We do have set rates for various types of appointments.  Please call the office if you would like to learn more.

Well that is a tricky one!  Everyone has different types of insurance with various set deductibles, co-pays, or set percentages their insurance dictates they pay.  This is where understanding the insurance policy you sign up for is extremely important.  If you need help with what your selected insurance covers or pays for, we can help or call your plan administrator/ insurance company.
